4. Minimum Six (06) Months Valid Passport with Old Passport

9. United States Visa Application Form.
10. TAX Certificate

For Business Person:

1. Company Bank Statement.
2. Company Related Documents.

For Job Holder:

1. NOC / Forwarding Letter.
2. Office ID card copy.
